What is a Summary Care Record?
Managing your health record is important to us. To support Patient care, all patient information is stored electronically in a Summary Care Record (SCR). It is a copy of key information held in your GP record that includes past and present conditions, current medications and any allergies that you have. This provides authorised healthcare staff with faster, secure access to essential information about you. This is especially helpful when you need unplanned care elsewhere or when your GP practice is closed. Confidential access to this information will improve the safety and quality of the care you receive.
You can choose to have an SCR, or you can choose to opt out.
If you choose to have a Summary Care Record and are registered with The Bell Surgery, you do not need to do anything; a SCR will have been created for you. However if you choose to opt out you need to let us know by filling in and returning the opt-out form. This will ensure that your record is not shared with other NHS services.

Sharing information across the NHS helps ensure that the quality and safety of services is consistent throughout the country. It can also highlight different diseases and conditions that may require more NHS investment. The care.data programme gives an opportunity for each of us to help the NHS provide the highest standard of care for all.

You have a choice: If you are happy to for your information to be used in this way, you do not have to do anything. If you have any concerns, or wish to prevent this, please speak to practice staff or download the opt-out form, complete it and return it to Reception Care.Data opt-out form
